Fees are based on reasonable and customary charges for this area. An estimated fee for any procedure will be given on request by our office. All co-payments are expected at the time of your visit. If you are unable to pay in full at the time of service, prior arrangements MUST be made with our Financial Counselor BEFORE services can be rendered. If your account is covered by insurance, all co-payments and deductibles must be met, and payment is expected within 60 days from your insurance company.

If you have a financial problem, we will understand. Contact the Financial Counselor, time will be extended for payment which is fair to you and the clinic. If you do not make these arrangements, your account is due 60 days after service.
